Job Title:           Nurse Practitioner (Advanced Registered Nurse Practitioner)

Salary:                $80,000 minimum

 

Job Summary

Under the supervision of the Medical Director, and in coordination with the Center Manager for administrative functions, the Nurse Practitioner (ARNP) is responsible for:  assessing, coordinating, monitoring, and providing health care services and treatment for CEC participants.  May supervise 1-3 health care employees.

 

Essential Job Functions

·        Conducts initial medical history, comprehensive physical examination, and functional nursing assessment of potential participants, and performs follow up medical assessments at least every six months.

·        Develops and implements participant medical care plans in conjunction with the Primary Care Physician, records progress notes, and assists in provision of continuity of care through routine monitoring and treatment of chronic medical problems.

·        Administers or orders diagnostic tests, and performs therapeutic procedures such as injections, immunizations, wound care, and managing infections.

·        Instructs and counsels participants regarding compliance with prescribed therapeutic regimens, medicine usage, and health maintenance per Physician’s orders.

·        Provides liaison with primary care provider in the event of an episodic illness; assists in coordinating services provided by primary care provider.

·        Serves as a member of the Interdisciplinary Team (IDT) and participates in the development of the plan of care.  Provides updates to team regarding participant’s change in health or functional status.

·        Monitors routine nursing care on a regular basis; may provide supervision to nurses or other health care personnel.

·        Assists participants in maintaining optimum health; provides health education and counseling to participants and caregivers facing chronic conditions and end of life issues.

·        Maintains confidentiality of participant information.

·        Attends and participates in staff meetings, in-services, projects, and committees as assigned.

·        Must be flexible in schedule of hours worked, including on-call coverage on a rotating basis.

·        Performs other duties as assigned.

 

Non-Essential Job Functions

·        Identifies problematic trends and/or service delivery outcomes not meeting established standards of care.

·        Assists in the monitoring of contracted medical providers for compliance with standards of care.

·        Adheres to and supports the facilities’ policies, practices, and procedures, and professional ethical standards.

·        May require use of personal vehicle.

Ability to access all areas of the facility throughout the workday. Ability to regularly lift and/or move up to 25 pounds and occasionally lift and/or move more than 100 pounds. The ARNP is required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds. Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.

 

Variable working conditions (ADHC, hospital, nursing home, at home or elsewhere when on call).  Comfortable work atmosphere.  Some undesirable conditions at ADHC – exposure to odors, fumes, infections, dust, dirt, which may be objectionable.  Local car travel occasionally necessary.  Out of town travel is minimal.

 

Knowledge, Skills and Abilities

·        Minimum one (1) year recent successful experience working with a frail or elderly population; work experience as an NP in a community-based or long-term care setting preferred.

·        Working knowledge of physical, mental and social needs of the frail elderly population.

·        Basic computer skills required; moderate or better computer literacy preferred.

·        Ability to work independently and as a member of a team.

·        Ability to relate to the public and governmental agencies in a pleasant, patient, and professional manner.

·        Ability to work effectively and in a collegial manner with all members of the Interdisciplinary Team, the participants, and caregivers.

·        Effective oral and written communication skills.

 

Education/Certification

·        Education:  Bachelor’s Degree from 4-year college or university, or must have graduated from an accredited Nurse Practitioner’s program which meets the Oklahoma State requirements for licensure.

·        Licensure:  Oklahoma State Nursing License

·        National Nurse Practitioner Certification (Geriatric specialty or similar preferred).
