Job Summary
Under the direction of the Manager of Transportation, responsible for providing transportation utilizing specified Cherokee Elder Care vehicles to all Cherokee Elder Care participants. Provides all delivery services as needed. Assists in Facilities Maintenance duties including preventive maintenance, minor repairs of systems and equipment, landscaping, custodial activities, and bio-hazard material disposal.
Essential Job Functions
· Transporting participants between home, Cherokee Elder Care, medical appointments and Cherokee Elder Care activities.
· Responsible for the safety of passengers from time of pickup to destination. Escorts participants in and out of destination. Assures that participant is not left unattended, particularly in an unsafe situation.
· Assists participants in and out of the vehicles, providing physical assistance as necessary, in order to ensure comfort and safety.
· Participates as a member of the Cherokee Elder Care Interdisciplinary Team in ongoing participant assessment and monitoring. Reports any changes in the participant’s home or behavior to the Manager of Transportation or to other appropriate staff person as procedures indicate.
· Provides pick up and delivery services as required, including mail and meals.
· Responsible for the maintenance and safe operation of the vehicle, including:
- Compliance with maintenance and cleaning schedules per Cherokee Elder Care policy and procedure.
- Timely completion of required documentation/reports.
· Follows procedure for reporting accidents immediately after such incidents occur, whether vehicular or personal.
· Performs lawn care and landscaping duties as assigned.
· Maintains cleanliness of facility, inside and outside, as assigned, including sweeping, mopping, cleaning, trash disposal, etc.
· Performs cleaning and maintenance work related duties as assigned.
· Performs minor maintenance on mechanical, plumbing, and electrical systems.
· Participates in Quality Assessment and Performance Improvement (QAPI) activities as required.
· Reports to Cherokee Elder Care to assist in activities as requested.
Non-essential Job Functions
· Attends and participates in staff meetings, in-services, projects and committees as assigned.
· Adheres to and supports Cherokee Elder Cares’ policies, practices and procedures.
· Accepts assigned duties in a cooperative manner and performs all other related duties as assigned by the Manager of Transportation.
· Works scheduled hours, including overtime as required. Must be flexible in schedule of hours worked.
· Delivers durable medical equipment as needed.
· Assists participants with needs while on outings.
· Socializes with participants upon completion of work duties.
· Performs other duties as assigned.

While performing the duties of this job, the employee is regularly required to stand, walk, talk, hear, taste or smell. The employee frequently is required to use hands to finger, handle, or feel, and reach with hands and arms. The employee is occasionally required to sit, climb or balance, and stoop, kneel, crouch or crawl. The employee must frequently lift and/or move up to 50 pounds and occasionally lift or move more than 100 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
While performing the duties of this job, the employee is frequently exposed to outdoor weather conditions and occasionally exposed to the risk of electrical shock, and work near moving mechanical parts and vibration. The noise level in the work environment is usually moderate.
Knowledge, Skills and Abilities
· Minimum of two years successful driving experience required, preferably with an older adult, physically challenged population.
· Six months custodial experience required.
· Experience in minor automobile maintenance, electrical, plumbing, and lawn maintenance equipment preferred. Experience to include operation, preventive maintenance, and minor repair.
· Must have excellent communication skills, be able to cope with mental and emotional stress related to the position, function independently, have flexibility and personal integrity.
· Employee must demonstrate on an on-going basis the ability to develop and maintain good working relationships with co-workers and department heads.
Education/Certification
Education: High School Graduate or GED
Licensure: A valid Oklahoma Driver’s License, an accident-free driving record with no moving violations within the last two years. A MVR check will be required for this position.
